Physical Therapist (PT) – Full-Time

Post Rehab is hiring a Physical Therapist (PT) to become part of the rehabilitation team at The Elms Center. This full-time opportunity is ideal for a clinician who enjoys developing individualized treatment programs and helping residents restore movement, improve physical function, and achieve greater independence.

What You'll Do

• Perform physical therapy evaluations and establish individualized plans of care
• Provide skilled interventions addressing strength, balance, gait, transfers, endurance, and mobility
• Develop functional rehabilitation goals and track resident outcomes
• Modify treatment approaches based on clinical findings and resident progress
• Educate residents and caregivers on exercises, mobility strategies, and safety
• Maintain thorough and timely clinical documentation
• Coordinate care with PTAs, OT, SLP, nursing, and other healthcare professionals

Qualifications

• Active New Hampshire Physical Therapist license required
• Graduate of an accredited Physical Therapy program
• Strong clinical judgment, communication, and organizational skills
• Skilled nursing or post-acute care experience preferred, but not required
• New graduate Physical Therapists are encouraged to apply
