Make a Difference Beyond the Bedside
LPN Health Services Coordinator
Looking for an LPN role with more independence, variety, and purpose?
If you're an experienced LPN who enjoys coordinating care, building relationships, solving problems, and making a lasting impact, this may be the opportunity you've been searching for.
At Catholic Charities, you'll have the autonomy to manage your work, the responsibility to influence health outcomes, and the satisfaction of helping individuals live healthier, more independent lives. Every day brings new challenges, meaningful connections, and the opportunity to improve quality of life for the people we serve.
Our Mission and Core Values
Catholic Charities is a nonprofit organization serving Central Minnesota with core values rooted in Compassion, Hope, Accountability, Respect, Dignity, and Service.
Every day, we empower our neighbors and create impact in housing, food, and mental health services that is seen and felt for generations.
Why This Role Matters
This isn't a traditional nursing position.
You won't spend your day passing medications or working through an endless patient schedule. Instead, you'll serve as the healthcare leader for individuals living in our residential programs, helping ensure they receive the medical care, support, advocacy, and resources they need to thrive.
You'll work independently while collaborating with physicians, psychiatrists, pharmacists, families, guardians, managers, and direct support staff to coordinate quality healthcare services and improve health outcomes.
For the right person, this role offers the perfect combination of nursing knowledge, case coordination, training, problem-solving, and relationship building.
