We are seeking a compassionate and organized Social Services Designee to support residents in their emotional and social adjustment to long-term care. This role is vital in advocating for resident rights and enhancing their psychosocial well-being through assessments, care planning, and coordination with families and community resources.

Qualifications:

• Must meet state requirements for social service designation in long-term care

• Minimum 2 years of long-term care experience preferred

• Strong communication and organizational skills

• Experience in public relations and in-service training is a plus

• Social Services Certification is preferred

Key Responsibilities:

• Conduct psychosocial assessments and develop individualized care plans

• Maintain documentation and progress notes

• Coordinate referrals to community and social service agencies

• Support residents and families during transitions and care planning

• Participate in care conferences, MDS completion, and quality improvement initiatives

• Assist with admissions and provide emotional support to residents
