Location: Adrian, MI | Lynwood Skilled Nursing Facility

We are seeking a compassionate, motivated Physical Therapist to provide individualized treatment in a skilled nursing environment. This opportunity is ideal for a PT who enjoys working collaboratively with residents and an interdisciplinary care team.

What You’ll Do

• Evaluate residents’ functional abilities, mobility, balance, strength, and rehabilitation needs.
• Develop individualized treatment plans based on clinical assessments and resident goals.
• Provide therapeutic interventions to improve walking, transfers, coordination, and daily function.
• Monitor resident progress and adjust treatment approaches as appropriate.
• Document evaluations, treatment sessions, progress notes, and discharge recommendations accurately.
• Collaborate with nursing, occupational therapy, speech therapy, and other team members.
• Educate residents, families, and caregivers on exercises, mobility techniques, and safety strategies.
• Follow established clinical standards and applicable documentation requirements.

What We’re Looking For

• Current Michigan Physical Therapist license in good standing.
• Degree in Physical Therapy from an accredited program.
• Strong clinical judgment and effective communication skills.
• Ability to work independently while contributing to a multidisciplinary team.
• Previous experience in skilled nursing, post-acute rehabilitation, or geriatric care is preferred.
