Service Areas

This position covers the North and Northwest Chicagoland suburbs, including: Arlington Heights, Palatine, Mount Prospect, Des Plaines, Park Ridge, Glenview, Northbrook, Wheeling, Buffalo Grove, Schaumburg, Hoffman Estates, and surrounding communities.

What You'll Do

• Provide skilled nursing visits including wound care, medication management, post‑acute care, and patient education.

• Complete OASIS assessments (SOC, ROC, Recert, Discharge) within required timelines.

• Develop and update individualized plans of care in collaboration with physicians and interdisciplinary team members.

• Assess home environments for safety risks and document findings.

• Educate patients and caregivers on disease processes, medications, and symptom recognition.

• Communicate changes in condition promptly and obtain updated orders as needed.

• Document all care accurately and timely in the EMR.

• Coordinate care with PT/OT/ST and the broader clinical team.

• Manage a flexible caseload within assigned territories.

Requirements

• Active Illinois RN license.

• Minimum 1 year of nursing experience (home health preferred).

• Strong clinical judgment and ability to work independently.

• Reliable transportation.

• Experience with OASIS preferred.

• Familiarity with North/Northwest suburban communities is a plus.
