Coverage Area: Tallahassee, FL / Leon, Franklin, or Taylor Counties
(Territories designed to minimize drive time with clustered scheduling)

Compensation & Earning Potential

• Salary+ model: $110,000 – $117,000+ for 30 productivity points
• Pay-per-visit at $70+ per visit + Medicare differential pay for all visits over 30 points
• Quarterly bonus opportunities 
• Mileage reimbursement + data plan stipend

Your Role as a Home Health Physical Therapist

• Evaluate patients and develop individualized treatment plans 
• Provide therapeutic exercises and mobility training 
• Educate patients and caregivers on safety and recovery 
• Collaborate with physicians and interdisciplinary teams 
• Supervise Physical Therapist Assistants (PTAs) 
• Complete documentation using streamlined technology
