Physical Therapist (PT) – Full-Time

Post Rehab is seeking a Physical Therapist (PT) to join the rehabilitation team at Civita Care Center at Long Ridge. This full-time opportunity allows you to work directly with residents to improve movement, restore strength, and build greater independence while collaborating with an experienced post-acute care team.

What You'll Do

• Evaluate residents and develop individualized physical therapy plans of care
• Provide skilled treatment addressing mobility, gait, balance, strength, transfers, and endurance
• Establish functional rehabilitation goals and track resident progress
• Adjust treatment approaches based on resident needs and clinical response
• Educate residents and caregivers on exercises, mobility strategies, and safety techniques
• Maintain accurate and timely clinical documentation
• Partner with PTAs, OT, SLP, nursing, and other members of the interdisciplinary team

Qualifications

• Active Connecticut Physical Therapist license required
• Graduate of an accredited Physical Therapy program
• Strong clinical, communication, and organizational skills
• Skilled nursing or rehabilitation experience preferred, but not required
• New graduates are welcome to apply
