Dependable Hospice is seeking an experienced and compassionate RN Case Manager to join our growing Phoenix Hospice team.
This is an excellent opportunity for a dedicated nurse who enjoys building meaningful relationships with patients and families while providing exceptional end-of-life care.
As a Hospice RN Case Manager, you'll coordinate individualized care plans, manage patient needs, and collaborate with an interdisciplinary team to ensure comfort, dignity, and quality of life.
What You'll Do
Manage and coordinate care for an assigned caseload of hospice patients
Perform comprehensive nursing assessments and ongoing evaluations
Develop, implement, and update individualized plans of care
Provide symptom management, pain control, and comfort-focused nursing care
Educate and support patients, families, and caregivers
Collaborate with physicians, hospice aides, social workers, chaplains, and other team members
Ensure accurate, timely clinical documentation and regulatory compliance
Participate in interdisciplinary team meetings and care coordination
What We're Looking For
Current, unrestricted Arizona RN license
Hospice nursing experience preferred
Strong clinical assessment and symptom-management skills
Knowledge of hospice philosophy and end-of-life care
Excellent communication, organization, and time-management skills
Valid driver's license and reliable transportation
Compassionate, independent, and committed to patient-centered care
