First Choice Home Health provides every type of medical service that’s available for health care in a client's home. Starting with the basics, we offer caregivers who are trained in CPR and dementia care. Additionally, most of our caregivers are CNAs (certified nursing assistants) providing clients with that higher level of professional training and knowledge. We also provide transportation to and from appointments and errands for both clients and the community.
If a client's needs become greater requiring a registered nurse (RN), physical therapist (PT), occupational therapist (OT), and/or speech therapist (ST), we work with their doctor to provide this too. We also work with Medicare to deliver the medical equipment a patient needs to be cared for in the home, and offer a social worker for additional coordination assistance if needed.
Then, when the patient's doctor determines that he or she is well enough that they no longer qualify for Medicare-paid home health care, we’ll help the patient transition to our Medicare Part B outpatient therapy services, also performed in the home. No other health care agency in the area offers the continuity of care in the home that we do. So as a patient's medical needs change, they stay with us—same caregivers, nurses, and therapists. Clients prefer this because there are fewer phone calls and more familiarity with the caregiver team coming in and out of their home. This is how we're unique in our community.
